The most commonly used Massachusetts notary certificate. Required for deeds, powers of attorney, and most legal documents. Confirms the signer appeared in person, was identified through satisfactory evidence, and signed voluntarily for the document's stated purpose.
Massachusetts Jurat Form
Required for affidavits and sworn declarations. The signer must sign in the notary's presence and swear or affirm that the contents are truthful and accurate. Used for court filings, sworn statements, and legal affidavits.
Massachusetts Signature Witnessing Form
Used when a Massachusetts notary witnesses a signature without administering an oath or affirmation. Simpler than an acknowledgment. No voluntary statement required from the signer.
